Health department reports critical violations at locations throughout the district


Violations were detected at numerous locations

The Sandusky County Health Department’s Sanitation Division reported the following critical violations during food inspections between July 1 and August 1, 2024 https://www.yourfoodsafety.com/sandusky:

Bob Evans Restaurants, LLC, No. 319, 2011 Ohio 53, Fremont. Violation: Flies and mosquitoes observed throughout the facility. Drains are being cleaned and pest control is scheduled to come in July. The health department will re-inspect in one month.

Buffalo Wild Wings, 2200 Sean Drive, Suite L, Fremont. Violations: Wipe bucket with disinfectant less than 200 ppm. A new wipe bucket with a 200 ppm concentration was created; Spray bottles with cleaning chemicals and a wipe bucket with disinfectant were placed above the liquor in the bar area. Chemicals were moved under all food products during the inspection.

Burger King Restaurant #614, 2250 N. Ohio 53, Fremont. Violations: The ice maker in the back had a buildup of debris and needed to be cleaned; a chemical spray degreaser was on the wall above the coffee beverage sauces and needed to be removed. Follow-up inspection in August.

Casa Fiesta, 2455 Hayes Ave., Fremont. Violation: Some containers were discovered uncovered in walk-in refrigerators and freezers. The person at the exchange covered the containers. Milk crates containing onions were stored on the floor, which has been corrected.

Crystal Arbors Catering and Banquet Hall, 1800 E. State St., Fremont. Violation: Out-of-date food in refrigerator and walk-in cooler. The business had been closed for the past week and the refrigerators had not been cleaned. All expired food was discarded upon inspection.

Davidson Liquor Establishment LLC, 124 W. Madison St., Gibsonburg. Violation: Products are washed in the three-bin sink, which has no air gaps. It is recommended that gaps be installed to prevent backflow.

Denny’s #7939, 1750 Cedar St., Fremont. Violation: Sausage and bacon on a baking sheet in the refrigerator uncovered. Corrected during inspection.

DQ West, 1312 Oak Harbor Road, Fremont. Violation: A live cockroach was observed during inspection. Pest traps are in use. Will be followed up in one month.

El Aguila Bakery Inc., 1810 W. State St., Fremont. Violations: Employee moves baked goods from baking sheet to serving sheet with bare hands. Gloves must be worn. Saw raw pork stored over corn. Raw meat must be lower than fruits and vegetables and has been moved. Sink with attached hose does not have a backflow preventer. Corn husks are in a container in the sink with no air gap. Sink must be repaired and corn husks must be moved. Status of violations will be reviewed in one month.

Elks Club, 436 Croghan St., Fremont. Violation: The chlorine concentration in the dishwasher was 50 ppm. Dishwasher needs service.

Garcia Entertainment LLC, 400 N. Stone St., ground floor and patio, Fremont. Violation: Disinfectant concentration below 200 PPM. Concentration corrected during inspection.

Gibsonburg Truck Stop, 4541 US 20, Gibsonburg: Violations: No manager was present at the facility at the time of inspection. Ants and fruit flies near the donut display case. Pest control must be contacted. Will be inspected again in one month.

GL-CAP-Fremont, 1499 North River Road, Fremont. Violation: No measures in place to clean up vomit/diarrhea. An information sheet was distributed during the inspection.

HD Travel Center/HD Express Mart Inc., 1559 W. McPherson Highway, Clyde. Violations: Sanitizer did not register temperature at all. Rice was stored on stove at 90°F. Responsible person discarded rice. Food held warm for serving was 100°F and responsible person discarded that food during inspection. Bucker refrigerator in front of store was over 105°F. Refrigerator was moved for better air circulation. Several food items in walk-in refrigerator were 113°F and discarded during inspection. Food must be 105°F or lower. Walk-in refrigerator did not have a date label and responsible person discarded food during inspection. Food items must be dated. Track: No critical violations were detected.

Jimmy G’s BBQ, 1321 W. State St., Fremont. Violation: Cheesecakes and cream-based pies at 44 degrees. Pies were placed in refrigerator at 41 degrees or below.

JoJo’s Nite Club, 115 E. Main St., Woodville. Violation: No hand sink in kitchen. Owner plans to add a hand sink.

Kekis Cafe, 601 W. State St., Fremont. Violation: Disinfectant concentration was below 200 PPM. New disinfectant was made to meet standards.

Kohler’s 818 Club, 818 Croghan St., Fremont. Violation: Multiple food containers uncovered in the walk-in cooler and soup on the steam table uncovered. Employees had all containers uncovered at the time of inspection. The temperature of the food in the salad cooler was 48 degrees. The food had been in the cooler for one hour and was moved to the walk-in cooler. Follow-up action: Violations corrected.

Marco’s Pizza, 210 W. Main St., Woodville. Violation: Quaternary ammonium concentration was over 500 ppm. Person in charge diluted it with water at time of inspection and inspector determined an approximate water to quat ratio to achieve proper concentration while waiting for maintenance. No backflow preventer on hose in mop sink. Must be corrected.

Ohio Turnpike Blue Heron Hardees, 6410 CR 165, Genoa. Violation: Quaternary disinfectant concentration was too high at 500 ppm. Dispenser must be recalibrated.

Ohio Turnpike Wyandot Starbucks, 6410 CR 165, Genoa. Violation: Disinfectant level out of range, responsible person drained disinfectant and refilled sink. Spatula had sticky residue. Person at change had spatula washed and trained staff.

Panda Rex, 701 E. State St., Fremont. Violations: Food containers in walk-in refrigerator left uncovered. Open items were covered with plastic wrap or lids. Multiple food containers throughout the facility left unlabeled and dated.

Raevia’s Cafe, 640 N. Fifth St., Fremont. Violation: Eggs in refrigerator above ready-to-eat foods. Food moved to another shelf below ready-to-eat foods.

Sandusky Fuel LLC, 104 N. Broadway, Green Springs. Violation: The bone-in chicken for roasting was at a temperature of 113 degrees. The chicken was discarded at the inspector’s request. The temperature must not exceed 115 degrees.

Speedtrap Diner, 310 E. Main St., Woodville. Violation: Ice cream toppings/cake refrigerator reached 54 degrees. Most items were discarded. Repairs are being requested. Follow-up action: No critical violations.

Strand Theater and Backstage Lounge, 220 S. Front St., Fremont. Violations: The person in charge does not have the required health insurance for the location.

Surest Path Recover of Fremont, 825 N. June St., Fremont. Violation: Observation of pre-made coffee filters full of coffee grounds without lids.

The Garrison, 209 Garrison St., Fremont. Violations: Observed roast uncovered in walk-in cooler. Responsible person covered with plastic wrap at time of inspection. Small tabletop refrigerator containing cheese and deli meats observed reading between 50 and 55 degrees. Should be 41 degrees or less.

Pizza House, 120 W. McPherson Highway, Clyde. Violation: Spray bottle of cleaner without label. Bottle was labeled as a bottle to the on-site inspector.

Tim’s Bar and Grill, 445 Ohio Ave., Fremont. Violations: At the time of inspection, a handwashing sink full of towels was observed, obscured by hanging aprons. A spray bottle of cleaning chemicals was observed that did not have a common name on it. There is no prep sink in the facility for washing produce. The inspector will follow up in one month.

Valley View Health Campus, 1247 CR 176, Fremont. Violation: Chocolate ice cream was observed stored uncovered in a walk-in freezer with a cover placed over the item.

Wendy’s-Clyde, 1080 W. McPherson Highway, Clyde. Violation: Chicken patty was placed directly on refrigerator shelf. Person making change threw the chicken away and cleaned the shelf.

Whitey’s Diner, 216 E. State St., Fremont. Violations: Packaged and unpackaged foods – prevent contamination by separation, wrapping and segregation. Food not refrigerated on counter next to grill. Consumer Advisory: Missing asterisk identifying specific food item. Significant food debris on can opener blade and handle.

Windsor Lane Health Care, 355 Windsor Lane, Gibsonburg. Violations: Coffee grounds were pre-boiled in coffee filters and left on the counter. Coffee grounds were covered. No prep sink or air-gapped sink in the facility.
