Major recalls at Costco, Walmart, Canadian Tire, Toys R Us | News

According to Health Canada, many items are being removed from the shelves of popular stores in Canada due to serious health and safety concerns; some incidents have already been reported.

Recent recall alerts include an “Important Recall Notice” from Costco regarding Uriexo Vegicaps capsules (Costco item number 1658161) with NPN number 80095228 and strength of 500 mg D-Mannose, 75 mg Vaccinium macrocarpon (160:1.0, DHE: 9600 mg dry) Proanthocyanidins 9.0 mg, lots 220511, 23536A and 234001.

“Unauthorized combination of health products,” Health Canada’s recall warning said. “Lots may also contain an expired urinary tract infection test strip.”

Health Canada also warns buyers to “consult their healthcare practitioner before stopping use of the affected products or if there are any health concerns.”

Another recall affects certain tablets sold in Walmart stores in Canada.

Jamp Pharma is recalling Jamp Digoxin tablets (DIN 02498502, digoxin strength 0.0625 mg and batch number EG23DGX001) because they may contain oversized tablets.

“Return your JAMP Digoxin 0.0625 mg tablets to your pharmacy for an exchange as soon as possible. Until then, continue taking your JAMP Digoxin, but do not take any tablets that look oversized compared to your regular tablets,” the recall warning for buyers states.

“If you are not sure if your product is affected, ask your pharmacist. Contact a doctor immediately if you or a person you care for experiences serious side effects such as vomiting, confusion, vision problems, or heart problems.”

Another recall affects certain tools sold at Canadian Tire and Bass Pro Shops.

The C2 detachable crank tensioning aid, model number 2199, which can be used as an accessory with Excalibur arches manufactured after 2008 and some 2015 Stryker models, is being removed from shelves, Health Canada said in its recall alert to buyers.

“The hook may break, which could result in a piece of the broken hook striking the consumer, creating a potential impact hazard,” the warning states.

As of March 11, 2024, the company has received 146 reports of hook breakage incidents and no injuries in Canada, Health Canada said. Here is a photo of the crank tensioner.

In the United States, the company received 391 reports of broken hooks and one report of an injury caused by a broken hook.

The company said 3,922 units of the affected product were sold in Canada and 11,894 units in the United States from January 2008 to December 2021, Health Canada said.

“Consumers should immediately stop using the recalled product and contact Excalibur Crossbow Inc. for a replacement hook, ropes and instructions,” Health Canada said.

Another recall involves the Claire’s and Icing Stores Halloween witch hat sold at Toys R Us.

According to Health Canada, the product is being recalled due to flammability hazard.

The Halloween witch hat does not meet recognized flammability standards and therefore poses a risk of burns, the recall warning states.

As of June 24, the company had not received any reports of incidents or injuries in Canada or the United States, Health Canada said.

The company said 448 units of the affected products were sold in Canada and 5,700 in the United States from August 2023 to January 2024, Health Canada added.

“Consumers should immediately stop using the recalled product and return the Halloween witch hat to any Claire’s or Icing store for a full refund of the purchase price,” the warning states.

Some dumbbell sets sold at Best Buy are also being removed from shelves, according to a recall alert from Health Canada to customers.

The recall affects ProForm Rapid Strike 50-pound adjustable dumbbell sets, model PAMSDB20.

“Due to misalignment in the bottom of the molded tray, the weight plates may become detached from the handle during use, creating a risk of injury,” the warning states.

As of July 22, 2024, the company has received no reports of incidents or injuries in Canada. In the United States, the company has received eight reports of weight plates coming loose during use and three reports of injuries, including bruises and abrasions, Health Canada said.

The company said 651 units of the affected products were sold in Canada and 54,400 in the United States from October 2023 to July 2024, the warning said.

“Consumers should immediately stop using the recalled ProForm Rapid Strike 50-pound Adjustable Dumbbell Sets and contact the company to receive a free replacement molded tray,” Health Canada said in the statement.
