• Proof of residence in Will County (e.g. utility bill, mortgage statement, valid
rent)
• Provide proof of income for 30 days from all sources for all household members
• Printout of all services received from the Department of Human Services “Notice of
Decision letter”
• Applicant’s photo ID
• Social security cards for everyone in the household
• Must Have children under 16 years of age and can prove their age (date of birth
Certificate, school certificate or health card)
The holiday baskets include: A new coat, hat and gloves/mittens for each child in
Household; gifts for each child and food for your holiday dinner.
