Inspector’s Notes: Observation of TCS food in the walk-in cooler (milk, sausage and cheese sandwiches) held above 41°F. (COS-PIC discarded food.) Observation of chopped chicken, chicken quarters and butter in the walk-in cooler that dropped 1 degree Fahrenheit in 15 minutes and failed to cool to 41°F in 4 hours. Observation of removed tiles and flooring in the walk-in cooler and near the dishwasher. PIC was advised to ensure all flooring is smooth, cleanable and in good condition.

Inspector’s Notes: Observed a box of raw ground beef stored under raw chicken. Observed 2 bowls and a plate stored between clean dishes containing food debris. Observed meat slicers containing dried meat and oil behind the blade. Observed a sanitizing solution with a chlorine concentration of 0 ppm. Observed TCS foods (cheese slices in cooler at 48-50°F; lettuce and grilled chicken on salad bar, both at 48°F) kept cold at over 41°F. Observed sausage patties in microwave kept warm at 83°F.

Inspector’s Comments: Paper towels were not provided at the handwashing sink. (COS-PIC provided paper towels at the handwashing sink.) A 3-compartment sink with a disinfectant concentration of 0 ppm Quat was identified. (COS-PIC provided sinks with Quaternary Ammonium.) Deli ham, soup and boiled eggs were identified as being stored in the refrigerated section for more than 24 hours without a date stamp. PIC was advised to ensure that all food items stored for more than 24 hours are date stamped. Insects were identified in the facility. PIC was advised to ensure adequate pest control practices are in place to reduce pest occurrence.

Inspector’s Notes: Slimy substance observed on the ice machine divider in the kitchen area and on the ice machine divider attached to the beverage dispenser. Observed food being held hot at temperatures below 135°F. Observed employee without hair nets.

Inspector’s Notes: Observed employee health policies not being followed in the facility. Additionally, PIC was unable to describe the 5 symptoms and 6 illnesses that result in restrictions or exclusions in the facility. Observed employee beverages stored on the prep table. Observed condensation lines from the sushi cooler in the hand sink. Observed dishwasher sanitizing dishes with 0 ppm chlorine. Observed tempura vegetables and shrimp kept cold in the prep refrigerator at over 41°F. (COS-PIC disposed of food in trash.) Observed mixed vegetables and chicken kept warm on the grill at below 135°F. Advised PIC to ensure all TCS food is always kept warm at 135°F or above.
